Responsibilities include education, orientation, training, and quality control to ensure competency in electronic record systems and assessment tools.
Candidates must have a minimum of three years' experience as a home care case coordinator and possess a relevant health or human service degree. An active license to practice and knowledge of home care systems and policies are also required.
