Core responsibilities
The case worker will facilitate the development of individual case plans for adults with developmental disabilities and provide support services and crisis intervention. Responsibilities also include connecting clients to resources, educating them on life skills, and coordinating care with other team members.
Requirements summary
Candidates must have post-Secondary education and 1-3 years of experience working with individuals with disabilities or mental health needs. A valid driver's license and standard first aid/Cpr certification are also required.
