Job description
We're looking for a passionate and skilled Clinical Educator to join our team! in this role you'll support and mentor personal support workers (PSWs) through hands on training, skills development and ongoing education - both in office and throughout the Halton community.
You'll play a key part in ensuring high-quality, compassionate care by providing guidance, coaching, and practical instruction to staff working in home and community settings. Acclaim Health announces the availability of one (1) Full time Clinical Educator, Personal Support position. The incumbent will report directly to the Manager, Personal Support and will be responsible for the PSW Educational Support of the Home Care Department. The Educator is expected to travel to the office and off-site visits for trainings within the Halton Community. The individual will be required to work Monday to Friday from 9:00am – 5:00pm with the flexibility to work some evenings and weekends as needed.
Pay Rate
$63,756.00 - $69,673.50 per year
Primary functions
- Provides clinical consultation and guidance to PSWs using resources such as relevant research documents, best practice documents from partner agencies, the College of Nurses of Ontario and best practice guidelines
- Develops and conducts the employee orientation and oversees the preceptorship program for new PSWs
- Identifies clinical educational needs and oversees the development, coordination and evaluation of clinical education sessions for PSW services in collaboration with employees, Care Manager, Personal Support and the Director, Home Care Services
- Models and promotes patient service excellence within the PSW teams
- Provides group and 1:1 education sessions including delegating nursing acts to PSWs
- Participates in the interviewing process of PSW candidates
- Acts as the Acclaim Health lead for clinical research partnership (PSW-related)
- Implements and evaluates the PSW education plan using the principles of adult education
- Under the direction of the Manager, Personal Support, develops and implements quality initiatives aimed at correcting negative variances, addressing gaps, mitigating risks and promoting client safety
- Participates and collaborates with a multidisciplinary care team including staff, patients and caregivers, and funders in supporting the patient’s needs according to the patient’s care plan
- Identifies, minimizes and manages patient risk in accordance with risk management and incident reporting standards and policies
- Works in collaboration with the Service Coordination Specialists to ensure patient-focused scheduling
- Provides internal customer service excellence through leading by example utilizing coaching and mentoring to support employees in providing excellence in patient care
- Provides evening/weekend on-call service on a rotational basis
Qualifications
- & education:
- Diploma as Registered Practical Nurse
- Valid Ontario driver’s license, use of vehicle and insurance per Acclaim Health policy.
Experience & skills
- Minimum 2 to 3 years as a Registered Practical Nurse
- Demonstrated ability to coach, mentor and lead staff
- Strong verbal and written communication and interpersonal skills
- Effective verbal and written communication skills
- Demonstrated ability in assessment, problem-solving and negotiating skills
- Strong planning, coordinating and organizing skills
- Demonstrated knowledge and skills in quality/risk management
- Proven ability to multi-task and meet deadlines
- Proficient in Microsoft Word, Excel, Outlook and AlayaCare
- Knowledge of community resources
- Demonstrated ability to work independently and collaboratively as a team member
