The diabetes nurse educator provides culturally safe, client-Centered education and support to indigenous adults with type 2 diabetes and pre-Diabetes, focusing on developing self-Management skills and improving health outcomes. Responsibilities include individual counselling, group education, outreach, and participation in program monitoring and evaluation.
Candidates must possess an undergraduate degree in nursing and current registration with the college of nurses of ontario, along with a minimum of three years of relevant clinical experience in diabetes education or chronic disease management. Experience working with indigenous communities and a certified diabetes educator designation are considered assets.
