Youth Outreach Worker - SWAG

Carlington Community Health Centre is seeking a Youth Outreach Worker for its SWAG program. The position is temporary for a duration from September 2026 to July 2027 with the possibility of extension. The incumbent must be available to work 2:30 - 6:30pm, Monday through Thursday - afternoons and evenings and occasional weekends if required.

Position description

Are you passionate about empowering youth and building stronger communities? The Carlington Youth Program is seeking a dedicated Youth Outreach Worker (YOW) to join our team and make a difference for youth from vulnerable communities. Our program focuses on fostering self-development, community involvement, and academic success.

As a YOW, you will play a crucial role in maintaining and expanding services for SWAG participants (ages 12-18). You’ll deliver individual support and group activities, connect youth with academic and emotional support resources, and provide punctual case management and crisis intervention. Your work will also involve collaborating with other youth-serving agencies to coordinate community resources and expand youth programming.

PROGRAM RELATED ACTIVITIES • SWAG Program Delivery

• Develop and facilitate SWAG group activities and one-on-one support, promoting youth welfare, academic progress, and social development.

• Support: Ensure SWAG aligns with program objectives, balancing academic support, social and recreational activities, mentoring, healthy nutrition, and life skills development.

• Management: Conduct preliminary assessments, provide supportive listening, and crisis intervention; coordinate ongoing support with team members focused on case-management and individual support.

• Connection: Work with school staff, students, and parents to identify and address needs, linking them to academic, social, and emotional resources and identifying gaps in support.

• Planning: Plan, organize and facilitate workshops, outings, and community events tailored to the identified needs of participants.

• Involvement: Plan and support community youth activities, ensuring the smooth operation of the youth program and assisting team members as needed.

Position requirements

• Education
• Bachelor’s degree from a recognized university in a relevant discipline or equivalent combination of some post-secondary education and significant experience in Youth program development, implementation and group facilitation.

• Professional Experience
• 3-5 years of full-time experience working with vulnerable youth in community settings, providing support related to social, family, and academic challenges.
• Proven experience in organizing and facilitating group activities for youth.
• Experience in program development, implementation, monitoring, and evaluation, with a preference for youth or mentorship programs.
• Experience building networks and partnerships with community services and youth organizations.
• Knowledge of challenges faced by youth in culturally diverse, low-income communities and experience with marginalized populations.
• Background in health promotion, community development, and after-school or homework programs.
• Experience working in non-profit, community-based organizations.

• Key Competencies
• Understanding of group dynamics and facilitation techniques, particularly for culturally diverse youth.
• Knowledge and experience in crisis intervention principles, supportive counseling, group work, outreach, and mentorship.
• Skilled in event coordination, partnership-building, and engaging youth in sports and well-being activities.
• Familiarity with Ontario secondary school curriculum and strategies for supporting students with diverse educational needs.
• Strong knowledge of community resources for youth support, including emotional and social development services.
• Proficiency in case management, crisis intervention, and supportive listening techniques.
• Excellent communication skills and the ability to work effectively within a multi-disciplinary team.
• Computer literacy, including proficiency in Microsoft Office, client database software, and electronic record-keeping.
• Fluency in English is required

• Regular access to a vehicle and a valid driver’s license is an asset
