Requires a nursing degree or diploma and current registration as an RN with the College of Nurses of Ontario. Candidates should have 3-5 years of nursing experience in community, hospital, or public health settings, with a Diabetes Educator Certificate preferred.
The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team of providers includes physicians, nurse practitioners, registered nurses, social workers, health promoters, dietitians and physiotherapists.
1.0 FTE Registered Nurse Diabetes Education Program
The Registered Nurse will provide evidence-based education, and diabetes management support for individuals and their families with, or at risk for, type 2 diabetes.
Undergraduate degree in nursing from a recognized university, or combination of nursing diploma and additional relevant courses/training. Current Registration as a Registered Nurse (RN) with the College of Nurses of Ontario. Training and experience in diabetes/pre-diabetes education programs and support services. Diabetes Educator Certificate preferred. Three to five years nursing experience in a community setting or combination of community and hospital or public health settings. Experience in program development, implementation, monitoring and evaluation. Proficiency in the use of computers and various software applications. Strong interpersonal, public speaking and communication skills, including the demonstrated ability to explain complex health concepts to the lay public. Proficiency in French, is considered and asset. This is a fulltime position, working 37.5 hours per week at our West Lambton location in Sarnia. Experience will be taken into consideration on our 6-step wage grid. Vacation benefits, medical benefits, the Healthcare of Ontario Pension Plan and paid vacation are included. Primarily based out of the West Lambton, Sarnia site but travel to all sites may be required.
